But There Is Something More Important Than the Calculator
A calculator can tell you the difference between two times, but it cannot tell you whether those times are supported by the anesthesia record, cannot determine why there is a gap, cannot explain an overlap, cannot decide whether documentation supports the time being reported, and it cannot tell you which payer methodology applies unless you know which rules you are working under. That is why one principle runs throughout this entire collection, “Don’t start with the number of minutes, start with the record.”
Before finalizing anesthesia time, ask:
- Who provided the anesthesia service?
- When did anesthesia time begin and end?
- Are the documented times internally consistent?
- Are there separate segments, gaps, or overlaps requiring review?
- Does the calculation agree with the supported documentation?
- Which payer methodology applies?
Calculate only after you understand what the record supports.

One of my goals for Chart Talk has been to create more than educational articles. I want to build resources that help us think through the record.
